APPLICATION FOR ASSISTANCE
 
Our mission is to provide financial assistance for families who are unable to afford adaptive equipment, travel, therapy and other items that are not [fully] covered by insurance or other organizations. Qualifications include:

• Children under 21 years old
• Have mobility issues due to neuromuscular or degenerative disease
• Have a financial need

We expect families to actively fundraise for the majority of the costs and seek assistance from other charities and governmental agencies. Vehicle assistance is limited to Michigan residents.

Once families apply for assistance, the application will be assigned to a LV team member who will evaluate the family’s need, the child's situation, and their financial capacity. Supporting information and documents may be required and all information will be sent to the Board for assistance determination. The LV team member may also direct the family to other support resources.

Please note, we can not provide recommendations for vendors or assist in negotiating purchases.
